You must be at least 45 years old to complete the assignment and have some visible signs of aging. If you have completed a Lifestyle Lift assignment in the past then you are no longer eligible. Below is the information on the Lifestyle Lift assignment:
ØThis shop evaluates the call center when calling for the free brochure, the on-site initial free consultation, and the 48 hour follow up call that you should receive after your appointment. You will not actually have any procedure performed.
ØThere is a quiz you must pass and a confidentiality agreement you must agree to. After you read the instructions, email me and I will assign the quiz and send you the link for the online confidentiality agreement. You must complete the quiz and the agreement within 24 hours. Once you have completed both, then I will assign the shop to you.
ØOnce the shop is assigned you will call to set the appointment. You will then let me know what day your appointment is scheduled for as that is the scheduled shop date.
ØYour appointment must be at least 2 days later than the day you call to allow for a confirmation call back.
ØThis is a 2 part assignment. You will be assigned a second report to evaluate the 48 hour follow up call.
