Below is the information on the Lifestyle Lift assignment:

·Payment: $33 ($30/consultation + $3/follow up call)

·This shop evaluates the call center when calling for the free brochure, the on-site initial free consultation, and the 72 hour follow up call that you should receive after your appointment.  You will not actually have any procedure performed.

·There is a quiz you must pass and a confidentiality agreement you must agree to.

·Once the shop is assigned you will call to set the appointment.  You will then let me know what day your appointment is scheduled for as that is the scheduled shop date.
